What is Qubha's Hazard Hunting software?

It is a phone-first app for reporting and closing shop-floor safety hazards. Anyone reports an unsafe condition, unsafe act or near miss with photos, location and risk level. A supervisor or EHS assigns it to a responsible person with a target date. That person fixes it and submits root cause, preventive action and an "after" photo. An EHS verifier physically checks the fix and closes it - or sends it back for rework.

Every hazard keeps a full activity timeline, so the evidence for safety audits, ISO 45001 and customer visits is always ready. Built by Qubha Infotech, Chennai, for manufacturing plants in India. Works with Work Permit for high-risk jobs.

Why switch

A hazard written in a book is not a hazard fixed

Hazard register book or Excel

  • Operators don't report - the book is far away and nothing happens anyway.
  • "Oil near machine" - no photo, no exact place, no risk level.
  • Nobody owns the fix, and no target date is set.
  • Closed on paper, still unsafe on the floor.
  • Before an audit, the team rewrites the register.

With Qubha Hazard Hunting

  • Report from the spot - phone camera, four quick steps.
  • Photo, plant, line and machine - the fixer knows exactly where.
  • Owner and target date - overdue hazards flagged.
  • Before and after evidence, physically verified by EHS.
  • Audit-ready every day - timeline and dashboard always current.

  • Under a minute - four short steps.
  • Name the fixer - optional; goes straight to them.
  • Their own list - reporters follow what happened to their hazard.

FAQ

Hazard hunting questions

What is hazard hunting?

Hazard hunting is a shop-floor safety practice in which employees actively look for unsafe conditions, unsafe acts and near misses and report them so they are fixed before anyone is hurt. Its value depends on reporting being easy and on every report being corrected and closed.

What is hazard hunting software?

It replaces the hazard register book or spreadsheet. With Qubha Hazard Hunting, anyone reports a hazard with photos from their phone in under a minute; it is assigned to a responsible person with a target date, corrected with before and after evidence, and physically verified by EHS before it is closed.

Unsafe act, unsafe condition, near miss - what is the difference?

An unsafe condition is a hazard in the workplace, such as an open panel, oil on the floor or a missing guard. An unsafe act is something a person does unsafely, such as working without PPE or bypassing an interlock. A near miss is an event that could have caused injury or damage but did not. All three can be reported in the app.

How long does it take to report a hazard?

Usually under a minute. Four short steps: what you noticed, photos from the phone camera, where it is, and a few quick details such as type, risk level and category.

How is a hazard closed?

The responsible person records the action taken, root cause and preventive action and uploads an after photo or document. An EHS verifier physically checks the fix and either approves and closes the hazard or sends it back for rework with a reason.

What does the safety dashboard show?

Active, open, in-progress, awaiting-verification, overdue, high/critical and closed hazards; on-time closure percentage and average closure time; reports today, this week, this month and year to date; and charts by severity, department, category and location. Each tile opens the matching list.

How does it help in safety audits?

Safety and ISO 45001 audits look for evidence that hazards are identified, acted on and verified. Each hazard keeps its photos, location, risk level, corrective action, root cause, verification and a full activity timeline, so the record is ready whenever an auditor or customer asks.

Can we use it across several plants?

Yes. One company can have several plants, and each user sees only the plants they belong to, while management can view all of them.

Does it need an app install?

No. It works in any phone or desktop browser, and photos are taken directly with the phone camera.

Who makes it?

Qubha Infotech Pvt Ltd, Chennai, Tamil Nadu, India - the same team builds, installs and supports it.
